Alteplase for the management of occluded central venous access devices: safety considerations.
نویسنده
چکیده
Central venous access device (CVAD) dysfunction, with loss of patency because of thrombotic occlusion, has been reported to be the most common complication in central lines, even more common than catheter-related infection (Moureau, Poole, Murdock, Gray, & Semba, 2002). Occlusion of CVADs can cause life-threatening treatment delays, patient discomfort, infection, and the need for catheter removal and replacement. Alteplase (Cathflo Activase, Genentech, Inc.) is the only thrombolytic approved by the U.S. Food and Drug Administration (FDA) to restore catheter function as assessed by the ability to draw blood. Numerous studies have demonstrated the efficacy of alteplase for catheter clearance, along with a low incidence of complications (Semba et al., 2002; Shen et al., 2003). Although the incidence of complications is low, knowledge of safety precautions and considerations for special populations can help healthcare professionals to prevent unnecessary problems.

Treatment of occluded central venous catheters with alteplase: results in 1,064 patients.
PURPOSE Thrombosis of central venous access devices (CVADs) is a relatively frequent complication. Alteplase (tissue plasminogen activator) has been used to salvage dysfunctional devices. The purpose of this study was to analyze the safety and efficacy of alteplase after administration of a maximum of two 2-mg/2-mL doses to thrombosed CVADs. MATERIALS AND METHODS A combined analysis was perfo...
متن کاملRecombinant tissue plasminogen activator (alteplase) for restoration of flow in occluded central venous access devices: a double-blind placebo-controlled trial--the Cardiovascular Thrombolytic to Open Occluded Lines (COOL) efficacy trial.
PURPOSE Central venous access devices (CVADs) are a mainstay of current medical therapy but often become occluded by thrombus. Tissue plasminogen activator (alteplase), at a dose of 2 mg per 2 mL, has been shown to be effective in restoring flow to catheters proven by radiographic contrast injection to be occluded by thrombus. The purpose of this double-blind placebo-controlled multicenter tria...
متن کاملAlteplase for the treatment of central venous catheter occlusion in children: results of a prospective, open-label, single-arm study (The Cathflo Activase Pediatric Study).
PURPOSE Alteplase is approved for use in the restoration of function to occluded central venous access devices (CVADs); however, there are few prospective studies in children. This study was undertaken to evaluate the safety and efficacy of alteplase in the treatment of CVAD occlusions in a pediatric population. MATERIALS AND METHODS A prospective, multicenter, open-label, single-arm study ev...

thrombolytic therapy, an appropriate treatment option , if primary angioplasty is not available for the treatment of Acute Myocardial Infarction patients in the early hours after the onset of the symptoms. The aim of this study was to evaluate the safety and effectiveness of the tenecteplase drug vs alteplase in the treatment of STEMI patients. We searched the PubMed, cochrane library, Web Of ...
متن کاملAlteplase versus urokinase for occluded hemodialysis catheters.
BACKGROUND The use of central venous catheters as a source of vascular access in patients undergoing hemodialysis may be complicated by thrombosis. Frequently, thrombolytics are used in an attempt to reestablish blood flow through partially or completely occluded catheters. OBJECTIVE To compare the efficacy of alteplase (recombinant tissue plasminogen activator) versus urokinase in reestablis...
